Is your out-of-network acupuncture coverage subject to a deductible? If yes, is there an out-of-network deductible?
How much of that deductible remains this year?
What is the out-of-network out-of-pocket maximum?
How much of the out-of-network out-of-pocket maximum has been met so far this year?
Does your plan offer acupuncture benefits?
What conditions does the acupuncture benefit cover? (be sure to ask this question as it may not be readily disclosed.)
Is your coverage limited to a certain number of visits or a dollar amount each year?
How much of the acupuncture benefit has been used so far this year?
If you don’t have out-of-network benefits, be sure to notify your Human Resources representative to request their inclusion in the following year’s insurance plan offerings.
